OB Veterans Plaza Name Inscription Application

Thank you for considering a donation to the Ocean Beach Veterans Plaza Redevelopment. Below is the form to request placement of a veteran's name on the new plaza wall. There is a $250 tax-deductible donation that will be collected when submitting this application. This donation will be used for future maintenance of the new Veterans Plaza once constructed.


Parameters for Name Inscription

All names of veterans will be verified by the U.S. Department of Veterans Affairs for accuracy/service dates. The memorial wall inscriptions will have a uniform look designed to symbolize the camaraderie of service. Inscriptions will be limited to a total of 2 lines with 21 characters allowed on line one and 28 on line two. 

First Line: Veteran’s Rank and Name
Second Line: Branch of Service & Dates of Service
Medals and citations for valor in combat will be noted by military symbols etched next to the name


Order and Priority of Names Accepted

1) Ocean Beach and Point Loma ONLY (92107 and 92106) starting at the OB Street Fair, June 27, 2015
2) San Diego County ONLY starting January 1, 2016
3) All US Veterans starting March 1, 2016


Priority For Name Inscriptions Is In The Following Order

1) All names from the original Veterans Plaza
2) Medal of Honor recipients from across the country
3) Applicants who pay the one-time maintenance donation of $250 for plaza upkeep
4) Geographical location of applicant submitting form as stated above
5) KIA/MIA based on order/priority set above
6) Combat veterans based on order/priority set above
7)All other veterans  
*Exclusion of any veterans dishonorably discharged will be on a case-by-case basis*


Maintenance Donation

A one-time maintenance donation of $250 will be charged upon application submittal to be used solely for plaza upkeep.

Thank you for applying to have a veteran’s name inscribed on the Veterans Plaza. A confirmation email will be sent to confirm your request. Your donation of $250 for the maintenance of Veterans Plaza is tax deductible with Tax ID # 33-0583777.
